A Community-Governed Infrastructure for Whole-Person Care
The Chicago Regionwide Community Information Exchange (CIE) is a network of providers working together to coordinate whole-person care across organizations and sectors, supported by shared infrastructure that helps partners exchange information while continuing to use the systems they already have in place.
It brings together relevant health and social information from participating systems, helps match that information to the right person, and supports communication and coordination across organizations and sectors.
Why Did We Build the CIE?
People with complex health and social needs receive services across multiple healthcare, behavioral health, housing, social service, and community-based organizations.
But the information needed to coordinate that care is often fragmented across disconnected systems that do not readily share information with one another. Providers may have limited visibility into the services a person is receiving across organizations, making it harder to coordinate care, avoid duplicative work, and respond to the range of a person’s needs.
What Are the Benefits of the CIE?
For People
The CIE reduces the burden on people to retell difficult experiences and remember every detail of their care history. People no longer have to be the coordinators of their own care across systems and organizations.
For Organizations
The CIE helps member organizations better understand the people they serve, and it enables partners to communicate and coordinate across organizations and sectors. This helps them reduce duplicative work, manage referrals and follow-up, identify unmet needs, and make more informed decisions about the services and supports a person needs.
For community-based organizations without a case-management system, the CIE can serve as their own system to document services, manage referrals, and coordinate care.

The CIE provides shared infrastructure for organizations and providers to coordinate around health-related social needs such as housing, food and nutrition, behavioral health, transportation, legal and financial aid, maternal and child health, and other interconnected needs.
The CIE supports coordination models aligned with organizational workflows and the needs of the people they serve, using a shared foundation for governance, consent, information sharing, referrals, and cross-sector coordination to improve health outcomes.
