Your Data. Your Decision.

It's your decision whether providers in the Chicago Regionwide Community Information Exchange® (CIE®) can share your data with one another.

You have the ability to opt out of providers sharing your data within the CIE. You can also opt back in later, in case you change your mind. The list of questions and answers below explain how you can manage your data.

  • The Chicago Regionwide CIE uses an opt-out model. This means that your information related to accessing services may be shared across the CIE network unless you choose to opt out. You have the right to decline participation at any time and it will not affect your ability to receive services.

  • Only authorized staff from CIE member organizations can view your information, and only for purposes related to coordinating your care and connecting you with services through the CIE.

  • Your information may be shared across the CIE network to help providers work together, coordinate services, and support your needs. Your information will be kept confidential and used only for these purposes. 

    Some providers may still ask you to sign a separate consent or authorization form depending on the type of information being shared, in accordance with federal, state, and local privacy laws, including HIPAA. 

    You have the right to opt out at any time. If you choose to opt out, your information will no longer be shared through the CIE, except as permitted or required by law. 

  • Your privacy is extremely important to the CIE. The CIE uses secure technology and processes to protect your information and complies with state and federal privacy and security laws, including HIPAA.

  • Send your privacy-related questions to ellen.clewett@iphionline.org.

  • You can opt out of sharing your information through the CIE at any time by filling out the CIE Opt-Out form and emailing it to ellen.clewett@iphionline.org. Alternatively, you can fill out the opt-out form online. See more below.

    Once your opt-out request is processed, your information will no longer be shared across the CIE network.

    If you don’t want a specific provider to collect your information in the first place, please contact them directly to discuss your options for opting out.

  • The CIE Opt-Back-In form allows you to opt back in to sharing your information through the CIE after you have previously opted out of the CIE.

    Just fill out the provided Opt-Back-In form and send it to ellen.clewett@iphionline.org.
