It's your decision whether providers in the Chicago Regionwide Community Information Exchange® (CIE®) can share your data with one another.

You have the ability to opt out of providers sharing your data within the CIE. You can also opt back in later, in case you change your mind. The list of questions and answers below explain how you can manage your data.

How to Opt Out?

The CIE Opt-Out form allows you to opt out of sharing your information through the CIE. Just fill out the provided Opt-Out form and send it to ellen.clewett@iphionline.org.

How to Opt Back In?

The CIE Opt-Back-In form allows you to opt back in to sharing your information through the CIE after you have previously opted out of the CIE. Just fill out the provided Opt-Back-In form and send it to ellen.clewett@iphionline.org.
